REQUIRED FORMS
Most of our school organized activities and sports require a Participation Permission Form to be completed and returned to the office before participation in the activity may begin. Some coaches or advisers have additional contracts or permission letters required as well. Click on the activity link on this page to go the webpage for your particular activity/sport to find contracts and/or an email address for the coach/adviser.
All sports require that the student have a Physical Form on file with the school in accordance with the district health policy before participation in physical activity in that sport may begin.

LOW COST QUALITY HELMETS
NEED BIKE HELMETS?
If anyone there needs bike helmets, the order form is attached. They went out to schools today and don't want to leave you folks out.
These helmets are “TrueFit ™”, the newest technology, with straps that prevent slipping to better protect the occipital lobe. More people wearing helmets, however it has been determined that 1/3 of children wear it incorrectly. We can fix that! Please return the order form with a check payable to MSAD 72 by April 16 to your school office. They are a bargain at $8.99

We have access to low cost helmets through the Injury Prevention Center and Bell Sports. Order forms are available at the school office, attached to this message and as a link on the MSAD 72 web site www.msad72.k12.me.us Go to the Health & Wellness link (left) and click on Helmets 2009/2010. (This is an excel file, so you may need to open excel first). We must send a bulk order to the company from the school, so please return the order form to your school office and the check, payable to MSAD 72, by Tuesday, December 22, 2009. Helmets should arrive the week of January 4th.
The prices are very good. Go to www.giro.com to see photos and compare prices on line. Good basic helmets are Recruit 2 ($35), Sestriere ($45), Slingshot ($35). The G9 ($60) has a more advanced venting system and is available in the largest sizes (62-65 cm). The higher priced models have more sophisticated venting and other features.
A snow helmet must fit right and be worn properly. To determine the size you need, wrap a measuring tape around your head, being sure to locate the tape just above your ears and keeping the tape level front to back.
